I understand that if consent is withheld or revoked, I will not be able to use the Health Assistant.
• while steps have been taken that are designed to ensure the accuracy of the information provided by the Health Assistant, such information is obtained from a number of sources and may not be current.
• Information provided by the Health Assistant is intended for informational purposes only.It is not intended to make any medical diagnoses or drive diagnostic decision-making by a doctor, general practitioner or other health service provider. It is also not intended to constitute medical advice and is not a suitable substitute for medical advice obtained from your doctor, general practitioner or other health service provider.
